AOA体育平台 Senior Care Portland partners with the Southern Maine Agency on Aging, the Alzheimer鈥檚 Association and Partnership for Healthy Aging to offer educational programs and support groups designed to help older adults and their families cope with issues related to aging.
Our geriatric medicine specialists also provide care onsite in local nursing homes and assisted living facilities. This care coordination and clinical oversight helps older adults transition from one health care setting to another - such as an acute hospitalization to a skilled nursing facility. In addition to nursing home care and assisted living care, our geriatricians also see patients at New England Rehabilitation Hospital of Portland.
