Easy access to your health information, when it is needed most
Our program ensures that your health information is safely recorded and close at hand in the case of an emergency at home. Upon request, AOA体育平台 Emergency Medical Services Stephens Hospital team members will:
- Visit your home
- Help you fill out a health and medication form
- Place it inside a vial in your refrigerator
You will also receive a "Vial of Life" sticker to be displayed on a window or door at your home. The sticker alerts first responders that your health and medication information is available in a special vial in your refrigerator. The program is free and open to all community members.
